Transform Rez LogoWe can assist in your diabetes health. Ask for a TransformRez Care Card. Care Card feathers may be initialed by/at:

  • LL Diabetes Fitness Center Classes & Activites
  • Ball Club Fitness Center
  • IHS Eye Doctor
  • IHS Dentist/Dental Hygienist
  • Tribal Foot Care Nurse/Providers
  • IHS Podiatrist
  • IHS Pharmacist
  • Physical Therapy or Occupational Therapy (one initial per year)
